Scheduling Appointments

Athletic Training Room Hours:

9 am to 6 pm Tuesday through Friday, Monday and Saturday are appointment only.

Schedule may change due to athletic home contests.

Forms to Participate in Intercollegiate Athletics:

To participate in Herkimer College Athletics and to be able to schedule an appointment in the athletic training room you need the following:

- Valid health physical within the last year (click here)

- Herkimer College Medical Health Questionnaire (click here)

- Herkimer College Intercollegiate Athletics Statement of Insurance and Waiver/Release Form (click here)

- Copy of current insurance card
